This course will help learners recognize, reflect and respond to changes in condition in frail older adults in long-term care through the use of the Sensory Observation System (SOS) and SBAR techniques. Learners will understand apply these two systems to common clinical scenarios, and to transfer this learning to the daily practice setting. Anticipated outcomes of this course include enhanced information sharing, enriched appreciation for role interdependency and prevention of unnecessary emergency transfers.

The Ontario Centres for Learning, Research and Innovation in Long-Term Care (Ontario CLRI) enhance quality of care and living for residents across the province. Our Learning Division develops, delivers and evaluates educational resources and programs to train the current and future long-term care (LTC) workforce.

Criteria

This course is targeted to team and student Personal Support Worker (PSW’s), Registered Practical Nurses and Registered Nurses in long-term care.

Learning Objectives

Earners of this micro-credential are able to:

  • Recognize some of the common acute changes in condition for long-term care residents
  • Build a common understanding of the team’s role in early identification, prevention and response to acute changes
  • Perform systematic observations to identify and effectively communicate acute changes to the team (including the attending physician) and in the plan of care
  • Enhance team skills and values related to being self-reflective, relational-centered and proactive
  • Apply team communication and collaboration strategies
